By submitting this form, I (we) authorize Illiana Christian High School (Illiana) to initiate debit entries to my (our) Checking account at the deposit institution indicated on this form. This authorization is to remain in full force and effect until Illiana and my deposit institution have received written notification from me (or either of us) of its termination in such time and in such manner as to afford Illiana and my (our) deposit institution a reasonable opportunity to act on it.
